Invitation for quotation for supplying tools and equipment, 3rd round 2025 (kidney transplant surgery instrument set)

Ho Chi Minh City University of Medicine and Pharmacy Hospital invites interested suppliers capable of supplying tools and equipment, 3rd round 2025 (kidney transplant surgery instrument set) to submit quotations according to the information below.

Suppliers are requested to submit the electronic quotation file according to the instructions in the “Contractor quotation guide” section and send the stamped paper copy to the Equipment Supplies Department - 4th Floor – Area A – University of Medicine and Pharmacy Hospital (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City).

Contractor quotation guide:

Part 1. Submit quotation online

Step 1. Contractor downloads the Excel file of the quotation list here and fill in the data according to the template (contractors should not add or remove columns).

Step 2. Contractor clicks on the link here to submit quotation information.

Step 3. Contractor fills in the company name, company registration number, contact person name, phone number, contact email of the contractor.

Step 4. Contractor uploads the following files:

Upload scanned quotation file, signed and stamped at the "Scanned quotation file, signed and stamped" section: note the system only accepts pdf files.

Upload Excel quotation list file (file filled in Step 1) at the "Contractor quotation table" section: note the system only accepts excel files.

Upload capability documents, technical documents and other documents (if any) at the "Technical file, other files" section: note the contractor compresses all documents into a single archive (rar/zip).

Step 5. Select Save information and complete the quotation process.
If the contractor has changes in the quotation, please repeat the steps from Step 1 to Step 5.
Part 2. Submit hard copy to the Hospital
Contractor completes the data according to the template provided by the Hospital in the "Quotation form for contractors (hard copy form for contractor to sign and stamp)" section and attach the still-effective award contract for the items that have been awarded at medical facilities, technical documents of the goods (if any) to the Equipment Supplies Department - 4th Floor – Area A – University of Medicine and Pharmacy Hospital (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City)
