Invitation for quotation: Supply of tools and equipment for 2025 (2025 Examination Kit)

Ho Chi Minh City University of Medicine and Pharmacy Hospital invites interested and capable suppliers to quote for the supply of tools and equipment for 2025 (2025 Examination Kit) according to the information below.

Suppliers are requested to submit the electronic quotation file according to the instructions in the “Contractor Quotation Guide” section and send the stamped paper copy to the Equipment and Materials Department - 4th Floor – Area A - Ho Chi Minh City University of Medicine and Pharmacy Hospital (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City).

Contractor Quotation Guide:

Part 1. Submit quotation online

Step 1. Contractors download the Excel file of the quotation items here  and fill in the data according to the template (contractors note not to add or remove columns).

Step 2. Contractors click on the link here  to submit quotation information.

Step 3. Contractors fill in the company name, business registration number, contact person name, phone number, and contact email.

Step 4. Contractors upload the following files:

Upload a scanned quotation file with signature and seal in the "Scanned quotation file with signature and seal": note that the system only accepts PDF files.

Upload the Excel quotation items file (the file filled in Step 1) in the "Contractor's quotation table": note that the system only accepts Excel files.

Upload capability documents, technical documents and other files (if any) in the "Technical documents file, other files": note that contractors should compress all documents into a single archive (rar/zip).

Step 5. Click Save information to complete the quotation process.

If the contractor needs to modify the quotation, please repeat steps from Step 1 to Step 5.

Part 2. Send hard copy to the Hospital

Contractors complete the data according to the Hospital's provided template in the "Quotation template for contractors (hard copy template for contractor's signature and seal)" section and send the still-effective winning contract for the awarded items at medical facilities, technical documents of the goods to the Equipment and Materials Department - 4th Floor – Area A – Ho Chi Minh City University of Medicine and Pharmacy Hospital (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City)
