The University of Medicine and Pharmacy Hospital, Ho Chi Minh City invites interested and capable suppliers to provide quotations for supplying components and repairing medical equipment, 5th round 2025, according to the information below.

- Quotation form for contractors (hard copy form for contractor to sign and stamp): download.

Suppliers are requested to submit the electronic quotation file according to the instructions in the “Contractor Quotation Guidelines” section and send the stamped paper copy to the Materials and Equipment Department - 4th Floor – Zone A - University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City).

Contractor Quotation Guidelines:

Part 1. Submit quotation online

Step 1. Contractors download the Excel file of the quotation list here  and fill in the data according to the template (contractors note not to add or remove columns).

Step 2. Contractors click on the link here  to submit quotation information.

Step 3. Contractors fill in the company name, business registration number, contact person's name, phone number, and contact email.

Step 4. Contractors upload the following files:

Upload the scanned quotation file, signed and stamped, in the "Scanned quotation file, signed and stamped" section: note that the system only accepts PDF files.

Upload the Excel catalogue file (the file filled in Step 1) in the "Contractor quotation sheet" section: note that the system only accepts Excel files.

Upload capability documents, technical documents and other documents (if any) in the "Technical dossier file, other files" section: note that contractors should compress all documents into a single archive file (rar/zip).

Step 5. Choose Save information and complete the quotation process.

In case the contractor makes changes to the quotation, please repeat steps from Step 1 to Step 5.

Part 2. Send hard copy to the Hospital

The contractor completes the data according to the hospital-provided template in the "Quotation form for contractors (hard copy form for contractor to sign and stamp)" section and includes the valid award contract for the awarded items at medical facilities, technical documents of the goods (if any) to the Materials and Equipment Department - 4th Floor – Zone A – University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City)
