Invitation to submit quotation for Supply of X-ray and Gamma radiation dose rate meters

The University of Medicine and Pharmacy Hospital, Ho Chi Minh City invites interested and capable suppliers to provide quotations for the supply of X-ray and Gamma radiation dose rate meters according to the information below.

- Quotation form for contractors (hard copy form for contractor signature and seal): download.

Suppliers are requested to send the electronic quotation file according to the instructions in the “Contractor Quotation Guide” section and to send the stamped paper copy to the Equipment and Materials Department - Floor 4 – Area A – University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City).

Contractor quotation guide:

Part 1. Submit quotation online

Step 1. Contractors download the Excel file of the quotation list here  and fill in the data according to the template (contractors note not to add or remove columns).

Step 2. Contractors click the link here to submit quotation information.

Step 3. Contractors fill in the company name, business registration number, contact person name, phone number, and contact email.

Step 4. Contractors upload the following files:

Upload a scanned quotation sheet, signed and sealed, in the "Scanned quotation file, signed and sealed" section: note that the system only accepts PDF files.

Upload the Excel list file (the file filled in Step 1) in the "Contractor quotation sheet" section: note that the system only accepts Excel files.

Upload capability documents, technical documents and other documents (if any) in the "Technical file, other files" section: note that contractors should compress all documents into a single archive (rar/zip).

Step 5. Choose Save Information and complete the quotation process.

If the contractor needs to make changes to the quotation, please repeat steps from Step 1 to Step 5.

Part 2. Send hard copy to the Hospital

The contractor completes the data according to the Hospital's template in the "Quotation form for contractors (hard copy form for contractor signature and seal)" section and sends it together with the valid winning contract for the awarded items at medical facilities, technical documentation of the goods (if any) to the Equipment and Materials Department - Floor 4 – Area A – University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City).
