Invitation for quotation for Supply of medicines at Pharmacy Facility 1, 2025 (Round 1)

University of Medicine and Pharmacy Hospital Ho Chi Minh City invites interested and capable suppliers to provide quotations for supplying medicines at Pharmacy Facility 1, 2025 (Round 1), according to the information below.

Suppliers are requested to submit the electronic quotation file according to the instructions in the “Contractor quotation guidelines” section and send the signed paper copy to the Pharmacy – Department of Pharmacy – Ground Floor – Area A – University of Medicine and Pharmacy Hospital Ho Chi Minh City (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City).

Contractor quotation guidelines:

Part 1. Submit quotation online

Step 1. Contractors download the Excel file of the invitation list here and fill in the data according to the template (contractors should not add or remove columns).

Step 2. Contractors click the link here to submit quotation information.

Step 3. Contractors fill in company name, business registration number, contact person’s name, phone number, and email.

Step 4. Contractors upload the following files:

Upload scanned quotation file with signature and seal in the "File scan báo giá, có ký tên, đóng dấu" section: note that the system only accepts PDF files.

Upload the Excel catalog file (the file filled in Step 1) in the "Bảng chào giá của nhà thầu" section: note that the system only accepts Excel files.

Upload capability documents, technical documents and other documents (if any) in the "File hồ sơ kỹ thuật, file khác" section: note that contractors should compress all documents into a single archive file (rar/zip).

Step 5. Choose Save information to complete the quotation process.

If the contractor needs to modify the quotation, please repeat steps 1 through 5.

Part 2. Submit hard copy to the Hospital

Contractors complete the data according to the template provided by the Hospital in the "Biểu mẫu chào giá dành cho nhà thầu (biểu mẫu bản cứng nhà thầu ký tên, đóng dấu)" section and submit together with the award notice/decision valid until May 2025 for the awarded items at medical facilities, legal documents, contractor’s capability documents (showing business scope) to the Pharmacy – Department of Pharmacy – Ground Floor – Area A – University of Medicine and Pharmacy Hospital Ho Chi Minh City (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City).
