Invitation for quotation: Upgrade of DataCenter environmental management system
The University of Medicine and Pharmacy Hospital in Ho Chi Minh City invites interested and capable suppliers to submit quotations for upgrading the DataCenter environmental management system as per the information below.

- Quotation form for contractors (hard copy form for contractor to sign and stamp): download.
Suppliers are requested to submit the electronic quotation file according to the instructions in the “Contractor Quotation Guidelines” section and send the stamped paper copy to the Information Technology Department - 4th Floor – Area A – University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City).

Contractor quotation guidelines:
Part 1. Submit quotation online
Step 1. Contractors download the Excel file of the quotation list here and fill in the data according to the template (contractors note not to add or remove columns).
Step 2. Contractors click on the link here to submit quotation information.
Step 3. Contractors fill in the company name, business registration number, contact person name, phone number, and contact email.
Step 4. Contractors upload the following files:
Upload the scanned quotation file, signed and stamped, in the "File scan quotation, signed and stamped": note that the system only accepts PDF files.
Upload the Excel catalog file (the data-filled file from Step 1) in the "Contractor's quotation table": note that the system only accepts Excel files.
Upload capability documents, technical documents and other documents (if any) in the "Technical file, other files": note that the contractor should compress all documents into a single archive file (rar/zip).
Step 5. Choose Save information and complete the quotation process.
If the contractor needs to change the quotation, please repeat steps from Step 1 to Step 5.
Part 2. Submit hard copy to the Hospital
The contractor completes the data according to the template provided by the Hospital in the "Quotation form for contractors (hard copy form for contractor to sign and stamp)" and attaches the still-effective award contract for the items awarded at medical facilities, along with the technical documentation of the goods, to the Information Technology Department - 4th Floor – Area A – University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City)
