University Medical Center Ho Chi Minh City (UMC) is implementing an advanced cardiovascular technique: endomyocardial biopsy (EMB). This method enables early, accurate detection of myocardial disorders while playing a vital role in post-transplant surveillance for heart recipients.

According to Vu Hoang Vu, MD, PhD—Head of the Department of Interventional Cardiology at UMC—endomyocardial biopsy serves as a particularly valuable tool in identifying the underlying etiology of unexplained heart failure and assessing myocardial damage in complex pathologies. Obtaining a small heart tissue sample allows clinicians not only to reach an accurate diagnosis, but also to tailor individual treatment strategies for each specific patient.

Nguyen Xuan Vinh, MD, from the Department of Interventional Cardiology at UMC, stated that this technique is exceptionally critical for the surveillance of patients following heart transplantation. By directly obtaining tissue specimens from the heart, physicians can detect early signs of graft rejection, thereby promptly adjusting immunosuppressive regimens to preserve graft function. In addition, endomyocardial biopsy aids in diagnosing rare cardiac conditions such as giant cell myocarditis and cardiac amyloidosis—disorders that standard non-invasive imaging modalities often struggle to detect.

The endomyocardial biopsy protocol is meticulously standardized to ensure maximum patient safety. Physicians access the cardiac chambers through a venous route, utilizing a specialized bioptome to retrieve small myocardial samples under the guidance of advanced fluoroscopy and imaging equipment. The entire procedure is carried out in the Cardiac Catheterization Laboratory (Cath Lab) through seamless coordination among cardiologists, nurses, and technicians.

Although an invasive procedure, endomyocardial biopsy delivers substantial clinical benefits. At UMC, the process is rigorously controlled—from patient eligibility assessment to post-procedural surveillance—minimizing potential complications such as hemorrhage, cardiac arrhythmias, or valvular damage.

According to Au Thanh Tung, MD, Specialist Level II—Head of the General Planning Department—the application of endomyocardial biopsy opens up new avenues in the diagnosis and management of cardiovascular diseases. For post-heart transplant recipients, this technique facilitates early detection of allograft rejection for timely therapeutic intervention. In cases of unexplained heart failure, histopathological examination of myocardial tissue enables physicians to identify the exact nature of the injury, personalizing treatment plans and optimizing clinical outcomes.

Beyond serving as a definitive diagnostic modality, endomyocardial biopsy represents a significant milestone in interventional cardiology. At UMC, the synergy between high clinical expertise and cutting-edge equipment is establishing this procedure as an indispensable asset in cardiovascular care. With clear clinical indications and tangible efficacy, it holds great promise for long-term patient outcomes, contributing to elevating the standard of cardiovascular care in Vietnam.
