Suppliers are requested to submit the electronic quotation file according to the instructions in the “Contractor quotation guide” and send the stamped paper copy to the Building Management Department - B2 – Zone A - University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City).

Part 1. Submit quotation online
Step 1. Contractor downloads the Excel file of the quotation invitation list here and fills in the data according to the template (contractors note not to add or remove columns).
Step 2. Contractor clicks on the link here to submit quotation information.
Step 3. Contractor fills in the company name, business registration number, contact person name, phone number, and contact email.
Step 4. Contractor uploads the following files:
Upload the scanned quotation file, signed and sealed, in the "File scan quotation, signed and sealed": note that the system only accepts PDF files.
Upload the Excel catalogue file (the file filled in Step 1) in the "Contractor quotation sheet": note that the system only accepts Excel files.
Upload capability documents, technical documents and other documents (if any) in the "Technical file, other files" section: note that the contractor should compress all documents into a single archive (rar/zip).Step 5. Choose Save information and complete the quotation process.
If the contractor needs to modify the quotation, please repeat steps 1 to 5.
Part 2. Submit hard copy to the Hospital
The contractor completes the data according to the Hospital-provided template in the "Quotation form for contractors (hard copy form for contractor signature and seal)" section and submits it together with the valid winning contract for the awarded items at medical facilities, and the technical documents of the goods to the Building Management Department - B2 – Zone A) - University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City)
