Suppliers are requested to submit the electronic quotation file according to the instructions in the “Contractor Quotation Guidelines” section and send the sealed paper copy to the Building Management Department - Basement B2 – Area A – University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City).

Contractor Quotation Guidelines:
Part 1. Submit quotation online
Step 1. Contractors download the Excel bidding list file here and fill in the data according to the template (contractors note not to add or remove columns).
Step 2. Contractors click the link here to submit quotation information.
Step 3. Contractors fill in the company name, business registration number, contact person name, phone number, and contact email.
Step 4. Contractors upload the following files:
Upload a scanned quotation file, signed and sealed, in the "Scanned quotation file, signed and sealed" section: note that the system only accepts PDF files.
Upload the Excel category file (the data-filled file from Step 1) in the "Contractor's quotation sheet" section: note that the system only accepts Excel files.
Upload capability documents, technical documents and other documents (if any) in the "Technical documents file, other files" section: note that contractors should compress all documents into a single archive (rar/zip).Step 5. Choose Save Information and complete the quotation process.
If the contractor needs to modify the quotation, please repeat steps 1 through 5.
Part 2. Submit hard copy to the Hospital
The contractor completes the data according to the template provided by the Hospital in the "Quotation form for contractors (hard copy form for contractor's signature and seal)" section, signs and seals it, and submits it together with the valid winning contract for the awarded items at medical facilities, technical documents of the goods to the Building Management Department - Basement B2 – Area A – University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City).
