Invitation to Quote for Supplying Electrical Materials for Repair of Same-Day Surgery Unit and the Science and Training Department, 3rd Floor

The University of Medicine and Pharmacy Hospital in Ho Chi Minh City invites interested and capable suppliers to provide quotes for supplying electrical materials for repair of the Same-Day Surgery Unit and the Science and Training Department, 3rd floor according to the information below.

Suppliers are requested to submit the electronic quotation file according to the instructions in the “Contractor Quotation Guidance” section and send the stamped paper copy to the Building Management Department - B2 – Zone A - University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City).

Contractor Quotation Guidance:

Part 1. Submit quotation online

Step 1. Contractors download the Excel file of the invitation to quote list here  and fill in the data according to the template (contractors note not to add or remove columns).

Step 2. Contractors click on the link here  to submit quotation information.

Step 3. Contractors fill in the company name, business registration number, contact person name, phone number, and contact email of the contractor.

Step 4. Contractors upload the following files:

Upload scanned quotation file, signed and sealed in the "File scan báo giá, có ký tên, đóng dấu" section: note that the system only accepts PDF files.

Upload Excel catalogue file (the file filled in Step 1) in the "Bảng chào giá của nhà thầu" section: note that the system only accepts Excel files.

Upload capability documents, technical documents and other documents (if any) in the "File hồ sơ kỹ thuật, file khác" section: note that contractors should compress all documents into a single archive (rar/zip) file.

Step 5. Click Save information and complete the quotation process.
If the contractor needs to modify the quotation, please repeat steps from Step 1 to Step 5.

Part 2. Send hard copy to the Hospital

Contractors complete the data according to the Hospital-provided template in the "Biểu mẫu chào giá dành cho nhà thầu (biểu mẫu bản cứng nhà thầu ký tên, đóng dấu)" section and submit it together with the valid winning contract for the awarded items at medical facilities, technical documents of the goods to the Building Management Department - B2 – Zone A) - University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City)
