Invitation to submit price for supplying a computed tomography imaging system with photon counting technology
Ho Chi Minh City University of Medicine and Pharmacy Hospital invites interested and capable suppliers to submit price quotations for supplying a computed tomography imaging system with photon counting technology as per the information below.

Suppliers are requested to submit the electronic price quotation file according to the instructions in the “Contractor Price Quotation Guidelines” section and to send the stamped hard copy to the Equipment Supplies Department - 4th Floor – Area A – Ho Chi Minh City University of Medicine and Pharmacy Hospital (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City).

Contractor Price Quotation Guidelines:
Part 1. Submit quotation online
Step 1. Contractors download the Excel file of the bid list here and fill in the data according to the template (contractors should not add or remove columns).
Step 2. Contractors click the Submit Quotation button below to submit the quotation information.
Step 3. Contractors fill in the company name, business registration number, contact person name, phone number, and contact email.
Step 4. Contractors upload the following files:
Upload the scanned price list file, signed and stamped, in the "Scanned quotation file, signed and stamped" section: note that the system only accepts PDF files.
Upload the Excel bid list file (the file filled in Step 1) in the "Contractor's quotation table" section: note that the system only accepts Excel files.
Upload capability documents, technical documents and other documents (if any) in the "Technical file, other files" section: note that contractors should compress all documents into a single archive (rar/zip).
Step 5. Click Save Information to complete the quotation process.
If the contractor needs to modify the quotation, please repeat steps 1 through 5.
Part 2. Submit hard copy to the Hospital
The contractor completes the data according to the template provided by the Hospital in the "Price quotation template for contractors (hard copy template for contractor to sign and stamp)" section, signs and stamps it, and sends it together with the valid award contract for the items awarded at medical facilities, along with technical documents of the goods (if any) to the Administrative Office - 4th Floor – Area A – Ho Chi Minh City University of Medicine and Pharmacy Hospital (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City)
