Invitation to submit price quotation for supplying commemorative badges

Ho Chi Minh City University of Medicine and Pharmacy Hospital invites interested and capable suppliers to supply commemorative badges and submit quotations according to the information below.

- Price quotation form for contractors (hard copy form for contractor signature and seal): download.

Suppliers please submit the electronic quotation file according to the instructions in the “Contractor quotation guide” section and send the stamped paper copy to the Materials and Equipment Department - Floor 4 – Area A - Ho Chi Minh City University of Medicine and Pharmacy Hospital (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City).

Contractor quotation guide:

Part 1. Submit quotation online

Step 1. Contractor download the Excel tender invitation list here  and fill in data according to the template (contractors note not to add or remove columns).

Step 2. Contractor click on the link here  to submit quotation information.

Step 3. Contractor fill in company name, business registration number, contact person name, phone number, contractor's contact email.

Step 4. Contractor upload the following files:

Upload scanned quotation file, signed and sealed, in the "Scanned quotation file, signed and sealed" section: note the system only accepts PDF files.

Upload the Excel category file (data filled in Step 1) in the "Contractor's quotation table" section: note the system only accepts Excel files.

Upload capability documents, technical documents and other documents (if any) in the "Technical file, other files" section: note the system only accepts PDF files.

Step 5. Choose Save information and complete the quotation process.

If the contractor makes changes to the quotation, please repeat steps from Step 1 to Step 5.

Part 2. Send hard copy to the Hospital

Contractor completes the data according to the hospital's template in the "Price quotation form for contractors (hard copy form for contractor signature and seal)" section and includes the still-effective winning contract for the awarded items at medical facilities, technical documents of the goods to the Materials and Equipment Department - Floor 4 – Area A) - Ho Chi Minh City University of Medicine and Pharmacy Hospital (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City)
