Invitation for Bids for Supply of TVs and Signal Decoding Devices

The University of Medicine and Pharmacy Hospital, Ho Chi Minh City invites interested and capable suppliers to provide quotations for TVs and signal decoding devices as per the information below.

- Quotation form for contractors (hard copy form for contractor to sign and stamp): download.

Suppliers are requested to submit electronic quotation files according to the instructions in the “Contractor Quotation Guide” and send the signed hard copy to the Information Technology Department - 4th Floor – Zone A - University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City).

Contractor Quotation Guide:

Part 1. Submit quotation online

Step 1. Contractor downloads the Excel file of the bidding list here and fills in data according to the template (contractors note not to add or remove columns).

Step 2. Contractor clicks the link here to submit quotation information.

Step 3. Contractor fills in company name, tax code, contact person name, phone number, contact email.

Step 4. Contractor uploads the following files:

Upload scanned quotation file, signed and stamped in the "File scan báo giá, có ký tên, đóng dấu": note that the system only accepts PDF files.

Upload Excel bidding list file (file filled in Step 1) in the "Bảng chào giá của nhà thầu": note that the system only accepts Excel files.

Upload capability documents, technical documents and other documents (if any) in the "File hồ sơ kỹ thuật, file khác": note that contractors should compress all documents into a single archive (rar/zip).
