Ho Chi Minh City University of Medicine and Pharmacy Hospital invites interested and capable suppliers to submit price quotations for Supplying hand towels, toilet paper, according to the information below.

Suppliers please submit the electronic quotation file according to the instructions in the “Contractor Quotation Guidance” section and send the stamped paper copy to the Materials and Equipment Department - 4th Floor – Area A - Ho Chi Minh City University of Medicine and Pharmacy Hospital (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City).

Contractor quotation guidance:

Part 1. Submit quotation online

Step 1. Contractor downloads the Excel invitation list file  here  and fills in data according to the template (contractor should not add or remove columns).

Step 2. Contractor clicks on the link  here  to submit quotation information.

Step 3. Contractor fills in company name, business registration number, contact person name, phone number, and contact email.

Step 4. Contractor uploads the following files:

Upload scanned quotation file, signed and sealed in the “Scanned quotation file, signed and sealed” section: note that the system only accepts PDF files.

Upload Excel list file (the file filled in Step 1) in the “Contractor quotation sheet” section: note that the system only accepts Excel files.

Upload capability documents, technical documents and other documents (if any) in the “Technical file, other files” section: note that the contractor should compress all documents into a single archive (rar/zip).

Step 5. Click Save information and complete the quotation process.

If the contractor needs to change the quotation, please repeat steps 1 to 5.

Part 2. Send hard copy to the Hospital

Contractor completes the data according to the Hospital's template in the “Price quotation template for contractors (hard copy template for contractor signature and seal)” section and sends together with the valid winning contract for the awarded items at medical facilities, technical documents of the goods (if any) to the Materials and Equipment Department - 4th Floor – Area A - Ho Chi Minh City University of Medicine and Pharmacy Hospital (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City)
