Invitation for quotation for supplying inpatient cosmetics for 2026 round 1

Ho Chi Minh City University of Medicine and Pharmacy Hospital invites interested suppliers capable of supplying inpatient cosmetics for 2026 round 1 to submit quotations according to the information below.

- Quotation form for contractors (hard copy form for contractor signature and seal): download.

Suppliers are requested to submit the electronic quotation file according to the instructions in the “Contractor Quotation Guidelines” section and send the stamped hard copy to the Pharmacy Department - 4th Floor – Area A - Ho Chi Minh City University of Medicine and Pharmacy Hospital (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City).

Contractor quotation guidelines:

Part 1. Submit quotation online

Step 1. Contractor downloads the Excel file of the invitation list here  and fills in the data according to the template (contractors note not to add or remove columns).

Step 2. Contractor clicks the link here  to submit quotation information.

Step 3. Contractor fills in the company name, business registration number, contact person name, phone number, and contact email.

Step 4. Contractor uploads the following files:

Upload the scanned quotation file, signed and sealed, in the "File scan quotation, signed and sealed" section: note that the system only accepts PDF files.

Upload the Excel catalog file (the file filled in Step 1) in the "Contractor quotation table" section: note that the system only accepts Excel files.

Upload capability documents, technical documents and other documents (if any) in the "Technical file, other files" section: note that the contractor should compress all documents into a single archive (rar/zip).

Step 5. Choose Save information and complete the quotation process.

If the contractor changes the quotation, please repeat steps from Step 1 to Step 5.

Part 2. Submit hard copy to the Hospital

The contractor completes the data according to the hospital-provided template at the "Quotation form for contractors (hard copy form for contractor signature and seal)" section and includes the valid winning contract for the awarded items at medical facilities, technical documents of the goods to the Pharmacy Department - 4th Floor – Area A - Ho Chi Minh City University of Medicine and Pharmacy Hospital (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City).
