Invitation for Quotation: Provision of Service to Organize a New Trend Scientific Conference on Heart Valve Disease Treatment in 2025

The University of Medicine and Pharmacy Hospital, Ho Chi Minh City invites interested and capable suppliers to provide a quotation for the service of organizing a new trend scientific conference on heart valve disease treatment in 2025 according to the information below.

Suppliers are requested to submit the electronic quotation file according to the instructions in the “Contractor Quotation Guide” and send the stamped paper copy to the Science and Training Department - 4th Floor – Area A – University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City).

Contractor Quotation Guide:

Part 1. Submit quotation online

Step 1. Contractors download the Excel file of the quotation list here  and fill in the data according to the template (contractors note not to add or remove columns).

Step 2. Contractors click on the link here  to submit quotation information.

Step 3. Contractors fill in the company name, business registration number, contact person's name, phone number, and contact email.

Step 4. Contractors upload the following files:

Upload the scanned quotation sheet, signed and sealed, in the "File scan báo giá, có ký tên, đóng dấu": note that the system only accepts PDF files.

Upload the Excel catalogue file (the file filled in Step 1) in the "Bảng chào giá của nhà thầu": note that the system only accepts Excel files.

Upload capability documents, technical documents and other documents (if any) in the "File hồ sơ kỹ thuật, file khác": note that contractors should compress all documents into a single archive (rar/zip).

Step 5. Choose Save Information and complete the quotation process.

If the contractor makes changes to the quotation, please repeat steps from Step 1 to Step 5.

Part 2. Send hard copy to the Hospital

Contractors complete the data according to the Hospital's template in the "Biểu mẫu chào giá dànhcho nhà thầu (biểu mẫu bản cứng nhà thầu ký tên, đóng dấu)" section and submit it together with documents on capability, experience, technical specifications of the product, goods, services (if any) to the Science and Training Department - 4th Floor – Area A – University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City).
