Invitation for quotation: Supply of components and repair for Olympus soft endoscope equipment

The University of Medicine and Pharmacy Hospital, Ho Chi Minh City invites interested and capable suppliers to provide quotations for supplying components and repair for Olympus soft endoscope equipment according to the information below.

- Quotation form for contractors (hard copy form for contractor signature and seal):  download.

Suppliers are requested to submit electronic quotation files according to the instructions in the “Contractor Quotation Guidelines” section and send the sealed paper copy to the Materials and Equipment Department - 4th Floor – Area A - University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City).

Contractor Quotation Guidelines:

Part 1. Submit quotation online

Step 1. Contractors download the Excel file of the quotation list  here  and fill in the data according to the template (contractors note not to add or remove columns).

Step 2. Contractors click the link  here  to submit quotation information.

Step 3. Contractors fill in the company name, business registration number, contact person name, phone number, and contact email.

Step 4. Contractors upload the following files:

Upload a scanned quotation file, signed and sealed, in the "Scanned quotation file, signed and sealed" section: note that the system only accepts PDF files.

Upload the Excel catalogue file (data filled in Step 1) in the "Contractor quotation table" section: note that the system only accepts Excel files.

Upload capability documents, technical documents and other documents (if any) in the "Technical file, other files" section: note that contractors should compress all documents into a single archive (rar/zip).

Step 5. Click Save Information to complete the quotation process.

If the contractor needs to modify the quotation, please repeat steps from Step 1 to Step 5.

Part 2. Submit hard copy to the Hospital

Contractors complete the data according to the template provided by the Hospital in the "Quotation form for contractors (hard copy form for contractor signature and seal)" section and attach the valid award contract for the items awarded at medical facilities, as well as the technical documents of the goods, to the Materials and Equipment Department - 4th Floor – Area A) - University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City).
