Invitation for Quotation: Supply of Medical Equipment Repair Parts, 10th time, 2025

Ho Chi Minh City University of Medicine and Pharmacy Hospital invites interested suppliers capable of supplying medical equipment repair parts for the 10th time in 2025 to submit quotations according to the information below.

- The list of items for quotation approved by the Hospital: download

- Quotation form for contractors (hard copy form for contractor to sign and stamp): download.

Suppliers are requested to send the electronic quotation file according to the instructions in the “Contractor Quotation Guide” section and submit the stamped paper copy to the Materials and Equipment Department - 4th Floor – Zone A - Ho Chi Minh City University of Medicine and Pharmacy Hospital (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City).

Contractor Quotation Guide:

Part 1. Submit quotation online

Step 1. Contractor downloads the Excel file of the quotation list here  and fills in the data according to the template (contractors should not add or remove columns)

Step 2. Contractor clicks the link here to submit quotation information.

Step 3. Contractor fills in the company name, business registration number, contact person name, phone number, and contact email.

Step 4. Contractor uploads the following files:

Upload a scanned quotation file, signed and stamped, in the "Scanned quotation file, signed and stamped" section: note that the system only accepts PDF files.

Upload the Excel item list (file filled in Step 1) in the "Contractor quotation sheet" section: note that the system only accepts Excel files.

Upload capability documents, technical documents and other documents (if any) in the "Technical file, other files" section: note that contractors should compress all documents into a single archive (rar/zip).

Step 5. Choose Save Information to complete the quotation process.

If the contractor makes changes to the quotation, please repeat steps 1 to 5.

Part 2. Submit hard copy to the Hospital

The contractor completes the data according to the template provided by the Hospital in the "Quotation form for contractors (hard copy form for contractor to sign and stamp)" section and submits it together with the still-effective award contract for the items awarded at medical facilities, as well as technical documentation of the goods, to the Materials and Equipment Department - 4th Floor – Zone A – Ho Chi Minh City University of Medicine and Pharmacy Hospital (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City).
