Invitation for quotation to supply vaccines for vaccination services in 2025 (first round)

The University of Medicine and Pharmacy Hospital, Ho Chi Minh City invites interested and capable suppliers to provide quotes for supplying vaccines to carry out vaccination services in 2025 (first round) according to the information below.

- Approved quotation list of the Hospital: download

- Quotation form for contractors (hard copy form for contractor's signature and seal): download.

Suppliers are requested to submit the electronic quotation file according to the instructions in the “Contractor Quotation Guidelines” section and send the stamped paper copy to the Pharmacy Department - 4th Floor – Area A - University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City).

Contractor quotation guidelines:

Part 1. Submit quotation online

Step 1. Contractor downloads the Excel file of the quotation list here  and fills in the data according to the template (contractors note not to add or remove columns).

Step 2. Contractor clicks the link here to submit quotation information.

Step 3. Contractor fills in the company name, business registration number, contact person name, phone number, and contact email.

Step 4. Contractor uploads the following files:

Upload the scanned quotation sheet, signed and sealed, in the "Scanned quotation file, signed and sealed" section: note that the system only accepts PDF files.

Upload the Excel quotation list (the file filled in Step 1) in the "Contractor quotation table" section: note that the system only accepts Excel files.

Upload capability documents, technical documents and other documents (if any) in the "Technical file, other files" section: note that the contractor should compress all documents into a single archive (rar/zip).

Step 5. Click Save Information to complete the quotation process.

In case the contractor changes the quotation, please repeat steps 1 to 5.

Part 2. Send hard copy to the Hospital

Contractor completes the data according to the Hospital-provided template in the "Quotation form for contractors (hard copy form for contractor's signature and seal)" section and sends the award notice/decision valid until May/2025 for the winning items at medical facilities, legal documents, capability documents of the contractor (showing business scope) to the Pharmacy Department - 4th Floor – Area A - University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City).
