Invitation to Quote for Provision of Maintenance Services for Peplink Balance 2500EC Load Balancing Internet Device Including Firmware Upgrade License

Suppliers are requested to submit the electronic quotation file according to the instructions in the “Contractor Quotation Guide” section and send the stamped paper copy to the Pharmacy Department - 4th Floor – Area A - University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City).

Contractor Quotation Guide:

Part 1. Submit quotation online

Step 1. Contractors download the Excel file of the tender list here and fill in the data according to the template (contractors should not add or remove columns).

Step 2. Contractors click on the link here to submit quotation information.

Step 3. Contractors fill in the company name, business registration number, contact person name, phone number, and contact email.

Step 4. Contractors upload the following files:

Upload the scanned quotation sheet, signed and sealed, in the "Scanned quotation file, signed and sealed" section: note that the system only accepts PDF files.

Upload the Excel catalog file (the file filled in Step 1) in the "Contractor's quotation table" section: note that the system only accepts Excel files.

Upload capability dossier, technical documents and other documents (if any) in the "Technical dossier file, other files" section: note that contractors should compress all documents into a single archive file (rar/zip).

Step 5. Click Save to complete the quotation process.

If the contractor needs to modify the quotation, please repeat steps 1 through 5.

Part 2. Submit hard copy to the Hospital

The contractor completes the data according to the Hospital-provided template in the "Quotation form for contractors (hard copy form for contractor's signature and seal)" section and includes the valid award contract for the items awarded at medical facilities, technical documentation of the goods, and sends it to the Pharmacy Department - 4th Floor – Area A – University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City).
