Invitation to quote for provision of testing and calibration services for medical equipment

Ho Chi Minh City University of Medicine and Pharmacy Hospital invites interested and capable suppliers to provide testing and calibration services for medical equipment to submit quotations according to the information below.

- The approved quotation list of the Hospital: download
- Quotation form for contractors (hard copy form for contractor signature and seal): download.

Suppliers are requested to send the electronic quotation file according to the instructions in the “Contractor Quotation Guidelines” section and send the stamped paper copy to the Equipment Supplies Department - 4th Floor – Area A - University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City).

Contractor Quotation Guidelines:
Part 1. Submit quotation online
Step 1. Contractor downloads the Excel file of the quotation list here and fill in the data according to the template (contractors should not add or remove columns).
Step 2. Contractor clicks the Submit contractor quotation below to send quotation information.
Step 3. Contractor fills in company name, business registration number, contact person name, phone number, contact email.
Step 4. Contractor uploads the following files:
Upload the scanned quotation file, signed and sealed, at the "File scan quotation, signed and sealed" section: note that the system only accepts PDF files.
Upload the Excel quotation list (file filled in Step 1) at the "Contractor quotation table" section: note that the system only accepts Excel files.
Upload capability documents, technical documents and other documents (if any) at the "Technical dossier file, other files" section: note that the contractor should compress all documents into a single archive (rar/zip).
Step 5. Choose Save Information and complete the quotation process.
If the contractor needs to modify the quotation, please repeat steps from Step 1 to Step 5.

The contractor completes the data according to the template provided by the Hospital in the "Quotation form for contractors (hard copy form for contractor signature and seal)" section, signs and seals it, and submits it together with the valid award contract for the items awarded at the medical facilities, technical documents of the goods (if any) to the Administrative Department - 4th Floor – Area A – University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City)
