Invitation to quote for Supply of 24-hour blood pressure holter and ECG holter machines

The University of Medicine and Pharmacy Hospital, Ho Chi Minh City invites interested and capable suppliers to provide quotations for 24-hour blood pressure holter and ECG holter machines according to the information below.

Suppliers are requested to submit the electronic quotation file according to the instructions in the “Contractor quotation guidelines” section and send the stamped paper copy to the Equipment and Materials Department - 4th Floor – Area A – University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City).

Contractor quotation guidelines:

Part 1. Submit quotation online

Step 1. Contractors download the Excel file of the invitation list here  and fill in the data according to the template (contractors note not to add or remove columns).

Step 2. Contractors click on the link here to submit quotation information.

Step 3. Contractors fill in the company name, business registration number, contact person name, phone number, and contact email.

Step 4. Contractors upload the following files:

Upload the scanned quotation file, signed and sealed, in the "File scan báo giá, có ký tên, đóng dấu": note that the system only accepts PDF files.

Upload the Excel category file (the file filled in Step 1) in the "Bảng chào giá của nhà thầu": note that the system only accepts Excel files.

Upload capability documents, technical documents and other documents (if any) in the "File hồ sơ kỹ thuật, file khác": note that contractors should compress all documents into a single archive (rar/zip).

Step 5. Click Save information to complete the quotation process.

If the contractor makes changes to the quotation, please repeat steps from Step 1 to Step 5.

Part 2. Submit hard copy to the Hospital

Contractors complete the data according to the template provided by the Hospital in the "Biểu mẫu chào giá dành cho nhà thầu (biểu mẫu bản cứng nhà thầu ký tên, đóng dấu)" section and submit it together with the valid award contract for the items awarded at medical facilities, and the technical documents of the goods to the Equipment and Materials Department - 4th Floor – Area A – University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Cho Lon Ward, Ho Chi Minh City)
