Invitation to Quote for Supply of Equipment for Rehabilitation Department

University of Medicine and Pharmacy Hospital, Ho Chi Minh City invites interested and capable suppliers to submit quotations for Supply of equipment for the Rehabilitation Department according to the information below.

- Bid price form for contractors (hard copy form for contractor to sign and stamp): download.

Suppliers are requested to submit the electronic quotation file according to the instructions in the “Contractor bidding guide” section and send the stamped paper copy to the Equipment Supplies Department - Floor 4 – Area A – University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City).

Contractor bidding guide:

Part 1. Submit quotation online

Step 1. Contractor downloads the Excel file of the tender list here  and fills in data according to the template (contractors should not add or remove columns).

Step 2. Contractor clicks the link here  to submit the bid information.

Step 3. Contractor fills in the company name, business registration number, contact person name, phone number, and contact email.

Step 4. Contractor uploads the following files:

Upload scanned quotation file, signed and stamped in the "File scan báo giá, có ký tên, đóng dấu": note that the system only accepts PDF files.

Upload Excel quotation list (file filled in Step 1) in the "Bảng chào giá của nhà thầu": note that the system only accepts Excel files.

Upload capability documents, technical documents and other files (if any) in the "File hồ sơ kỹ thuật, file khác": note that contractors should compress all documents into a single archive (rar/zip).

Step 5. Choose Save information and complete the quotation process.

If the contractor needs to modify the quotation, please repeat steps 1 through 5.

Part 2. Send hard copy to the Hospital

Contractor completes the data using the template provided by the Hospital in the "Biểu mẫu chào giá dành cho nhà thầu (biểu mẫu bản cứng nhà thầu ký tên, đóng dấu)" section and sends it together with the valid award contract for the awarded items at medical facilities, technical documents of the goods (if any) to the Equipment Supplies Department - Floor 4 – Area A – University of Medicine and Pharmacy Hospital, Ho Chi Minh City (address 215 Hong Bang, Ward 11, District 5, Ho Chi Minh City).
