Guide to health insurance medical examination and treatment procedures for 2025
Ho Chi Minh City University of Medicine and Pharmacy Hospital (BV ĐHYD) is classified as a "Highly Technical Specialized" level according to Decision No. 3949/QĐ-BYT dated 27/12/2024, with a score of 94/100.
To ensure full health insurance benefits when receiving medical care at the Hospital, dear patients, please refer to the following guidance information, and present all required and correct procedures:
1. Health Insurance procedures:
a. Health Insurance card information (health insurance card/health insurance card number/CCCD with integrated health insurance information/VNeID that has integrated health insurance card information).
- For children under 6 years old who have not been issued a health insurance card: Present the original birth certificate or a copy or a birth extract / original or copy of birth certificate or citizen ID.
- For organ donors who do not have a health insurance card: Present the discharge paper issued by the medical facility where the body part was taken.
- For individuals participating in health insurance while waiting for card re‑issuance, card replacement or information about the health insurance card: When coming for examination and treatment, must present the receipt of file acceptance and appointment for receiving the results of issuance, re‑issuance and card replacement, health insurance card information provided by the social insurance agency or the organization/individual authorized by the social insurance agency to receive re‑issuance or replacement applications.
b. Present identification documents with a photo for health insurance cards without a photo, such as: National ID / citizen identification card / passport / VNeID / personal identification documents issued by the competent authority / police certification / certification from the educational institution managing the student. Excluding children under 6 years old.
c. Referral form the health insurance patient or the health insurance referral slip (abbreviated as referral document) must be transferred correctly to the University Medical Center Ho Chi Minh City for outpatient cases. Or an appointment slip for follow‑up visits in the case of re‑examination.
* In cases where the patient has a rare disease, a dangerous disease, a disease requiring surgery or the use of high‑tech services according to Appendix I of Circular 01/TT-BYT (attached), the patient should present disease proof documents such as prescription/discharge papers..., and a referral document is not required.

3. Some notes:
- Patients receive medical examination and treatment using the Referral Letter within 10 working days from the date of signing the Referral Letter.
- The Referral Letter transfers diseases according to Appendix III of Circular 01/TT-BYT (attached) which is valid for one year from the signing date, applicable to referral letters signed from the year 2024.
